What is Viagra (Sildenafil)
Sildenafil (Viagra) is a phosphodiesterase type 5 (PDE5) inhibitor. It is the first such oral medicine approved by the FDA in the United States . It is used for treatment of erectile dysfunction. It is not approved for women. Sildenafil 
inhibits PDE5, an enzyme that destroys cGMP and saves cGMP for a loner duration and lets it accumulate. cGMP extends the relaxation of the smooth muscle of the corpora cavernosa, which in turn increases the blood flow to the penis and causes erection of it. So, sildenafil can increase the blood flow to the penis and decrease blood going out from it. It helps in improving blood flow and thereby helps in erection of the penis in people who find it difficult to initiate and sustain an erection.
Is it effective?
Sildenafil is used for treatment of erectile dysfunction, which can be a result of physical or psychological causes. Sildenafil is effective against erectile dysfunction in men suffering from coronary artery disease, diabetes mellitus, hypertension, depression, coronary artery bypass surgery etc. It is also effective in men using antidepressants and anti-hypertensives. Sildenafil works fine with people who do not have any major health problems and helps them to get the necessary erection during sexual activity. It is effective and so is very popular among people suffering from erectile dysfunction or ED or impotence.
How should it be administered?
Sildenafil can be found in the form of oral tablets. These tablets are available in 25, 50 and100mg forms. Sildenafil should be taken one hour before sex. It may take 11-20 minutes to start working. It is to be taken on an empty stomach if you want the best results. On an empty stomach, absorption and effectiveness of the medicine is better. You should not take it after a meal high in fat.
Dose of sildenafil (Viagra)
To dose of sildenafil depends on age, health condition and other medication being taken by the patient. Doctors look into all these factors before prescribing sildenafil doses. The normal starting dose is 50mg. Doctors may increase or decrease the dose based on its effectiveness and side effects. The maximum recommended dose is 100mg to be taken every 24 hours. In general, most men will need 100mg of sildenafil for the highest effectiveness.
Aging, liver and kidney dysfunction, and concurrent use of certain medications (such as erythromycin -- an antibiotic, and protease inhibitors, for HIV) are some of the factors that can slow down breakdown of metabolism. In that case, sildenafil accumulates in the body and that can increase the risk of side effects. So, if the patient is over 65, suffering from substantial kidney and liver disease, and taking protease inhibitors, then doctors prescribed a lower starting dose of sildenafil- 25mg. A protease inhibitor ritonavir (Norvir) can highly increase the accumulation of sildenafil and so people taking Norvir should take only 25mg of sildenafil and that too once in 48 hours.
